Photos of the Food Truck:
- Please include current photos of the Food Truck/Trailer's interior and exterior.
Valid Vehicle Registration:
- If it is a Food Truck, it MUST have a trailer registration.
Copy of Automobile Insurance:
- A copy of the automobile insurance for the mobile food truck or main vehicle that will haul the food trailer.
Health Unit Permit:

Shreveport Caddo Tax Registration Certificate:
- Please contact the Shreveport Sales and Use Tax Commission at (318) 865-3312 for additional information.
Louisiana Sales Tax Registration Certificate:
Commercial General Liability Insurance:
- Coverage must be at least $500,000.
Hold Harmless Agreement:
- This form is included the application form.
Home-Based Business / Certificate of Occupancy:

Occupational License:
- Please contact our Plan Examiners at (318) 673-6442 to verify if an Occupational License is required.
Shreveport Fire Department Inspection:
